Grant Overview
Connecticut faces significant capacity gaps in tracking and analyzing chronic respiratory diseases, hindering effective program planning and resource allocation. The state's dense population and complex healthcare infrastructure require sophisticated data systems to identify trends and high-risk populations. Connecticut's existing health data infrastructure is fragmented, with multiple systems and stakeholders involved in data collection and analysis. To address this, grant funding will create a statewide data system to track chronic respiratory diseases. Infrastructure and Workforce Constraints: Connecticut's healthcare workforce is highly specialized, but lacks sufficient data analytics capacity to effectively utilize existing health data. The state's small geographic size belies its complex healthcare landscape, with multiple payers and providers requiring coordination. Readiness Requirements: Applicants must demonstrate experience with health data systems and analytics, as well as the ability to integrate with existing state health data initiatives. Connecticut's Health Information Exchange (HIE) will provide a foundation for this work. By facilitating comprehensive health data collection and analysis, this initiative aims to identify trends and high-risk populations, allowing healthcare providers to target interventions more effectively. The data system will be designed to meet Connecticut's unique demographic and geographic characteristics.
